  • The student’s latest report card or withdrawal form
  • The parent or guardian’s photo ID
  • Proof of residency: a current utility bill plus one more item, such as a lease, a purchase agreement, a tax return, a pay stub or a property tax bill
  • An official birth certificate
  • Georgia’s eye, ear and dental form and immunization form, available from any Georgia health department or pediatrician’s office
  • An enrollment disclosure form for grades 4 to 12

Ratings and test scores

Georgia rates schools with the College and Career Ready Performance Index (CCRPI), published by the Georgia Department of Education. The district links its latest CCRPI report from its accountability page. Search any school or district there for its scores in content mastery, progress, closing gaps, readiness and graduation rate. Georgia Milestones test results are on the state’s Milestones page.

When you compare schools, look at more than one year and at the parts of the score, not just the total.
